Pharmacy in Spain 2026: Farmacia de Guardia and What's Behind the CounterWesna Group

Spain has roughly 22,000 community pharmacies, about one for every 2,100 residents. For foreign buyers used to Boots or a 24-hour supermarket chemist, the Spanish farmacia looks familiar and behaves differently. Here is how it actually works in 2026.

Spain has roughly 22,000 community pharmacies, about one for every 2,100 residents, one of the densest networks in Europe (Consejo General de Colegios Oficiales de Farmacéuticos). On the Costa Blanca the ratio tightens in summer when the resident count triples. For a buyer used to a Boots inside Tesco or a Dutch drogisterij, the Spanish farmacia looks familiar from the outside and behaves differently once you walk in.

What does a farmacia actually do in Spain?

A Spanish farmacia is a licensed health establishment, not a retail convenience store. Only a titulado pharmacist can own one, and ownership is limited to one pharmacy per professional. That rule, defended repeatedly by the Tribunal Supremo, is why you will not see a Walgreens or a Rossmann chain here. Each shop is independent, run by its owner, and sits inside a numerus clausus system that caps how many farmacias can open in a given population zone.

What they sell: prescription medicines (receta), over-the-counter drugs (medicamentos sin receta), veterinary basics, infant formula, dermocosmetics, orthopaedic items and compounded preparations. What they do not sell: cleaning products, snacks, magazines, or most of the general merchandise a British chemist carries. For sunscreen, shampoo, nappies and toothpaste you can use a farmacia, but a supermarket or a parafarmacia will be cheaper. The parafarmacia, often sitting next door, is a non-prescription shop staffed by a technician, not a pharmacist, and it cannot dispense anything that requires a receta.

The green cross outside is not decorative. When it is lit, the pharmacy is open. When the digital display shows a scrolling address, that is the nearest farmacia de guardia on duty tonight.

How does the farmacia de guardia rotation and daily schedule work?

Spain guarantees 24-hour access to a pharmacy in every town through a rotating duty system. The local Colegio Oficial de Farmacéuticos publishes a monthly or quarterly calendar assigning each pharmacy its guardia shifts. During a shift, that pharmacy must be reachable overnight, on Sundays and on public holidays. In small towns the rotation can be weekly; in Torrevieja or Benidorm it is nightly across dozens of outlets.

Two practical points surprise foreign buyers. First, at night the door is usually locked. You ring the bell, speak through a hatch or small window, hand over the receta and cash, and receive the medicine. This is a security measure, not rudeness. Second, the guardia pharmacy may charge a small overnight supplement on non-urgent purchases, typically EUR 1 to EUR 3, which is legal and set by the regional Colegio.

You find the duty pharmacy three ways: the illuminated panel outside any closed farmacia, the Colegio website for your province (for Alicante, cofalicante.com; for Murcia, cofrm.es; for Valencia, micof.es), or by dialling 010 in most municipalities. Google Maps lists guardia status unreliably and should not be trusted at 2 a.m.

Outside the guardia rotation, Spanish pharmacies keep broadly similar hours, but they close for lunch and most of them close on Sundays. The split day is the single biggest adjustment for buyers from northern Europe.

Day Urban (Alicante, Murcia) Coastal and inland village
Mon to Fri morning 09:00 to 14:00 09:30 to 13:30
Mon to Fri afternoon 17:00 to 20:30 17:00 to 20:00
Saturday 09:30 to 14:00, some reopen 17:00 to 20:00 09:30 to 13:30 only
Sunday and holidays Closed except guardia Closed except guardia
August Reduced hours, some full closures Coastal: extended for tourists. Inland: 2 to 3 week closures

A minority of pharmacies in large cities operate horario ampliado (extended hours, usually 09:00 to 22:00) or horario continuo (no lunch break). In Alicante city there are a handful of 24-hour farmacias that do not rely on the guardia rota. Along the coast, the summer tourist flow pushes shops in Benidorm, Calpe and Torrevieja to stay open through siesta, while inland villages like Polop or Hondón de las Nieves follow the strict split day.

What needs a prescription in Spain, and who adapts to the system?

Spain is stricter than the UK or the Netherlands on what counts as a prescription-only medicine. Several items a British or Dutch buyer treats as kitchen-cupboard basics require a receta here.

Antibiotics: always prescription. Pharmacists have been fined and shops closed for dispensing without one. Do not ask.

Codeine, tramadol, strong painkillers: prescription only. Paracetamol and ibuprofen up to 400 mg are over the counter.

Benzodiazepines, sleeping tablets, most antidepressants: prescription with a controlled receta (receta de estupefacientes for some).

Contraceptive pill: prescription. The morning-after pill (levonorgestrel) is over the counter and the pharmacist will usually ask a few questions.

Insulin and diabetic supplies: prescription, but Spanish pharmacies will dispense an EU-issued prescription from any EEA country, which matters for Dutch, German and Nordic residents.

Asthma inhalers (salbutamol): prescription in theory, dispensed in practice in an obvious emergency at the pharmacist's judgement.

Residents with a tarjeta sanitaria receive prescriptions electronically (receta electrónica) and present the health card at the counter. The co-payment depends on income bracket and ranges from 10 percent to 60 percent for working residents, with a monthly cap. Pensioners pay 10 percent capped at EUR 8 to EUR 61 per month depending on income. Non-resident foreigners and private patients pay the full price, which for most common medicines is still cheaper than in the UK, Ireland or the US.

Buyers who speak Spanish or Valencian, and who accept the split day, adapt within weeks. Pharmacists are highly trained, they can advise on minor ailments without a GP visit, and in coastal towns many speak English, German or French. For a British retiree used to Boots, the biggest shift is remembering that nothing you need between 14:00 and 17:00 will be sold to you unless it is urgent and you walk to the guardia.

Buyers who struggle: those who assumed they could buy antibiotics casually, those relocating with ongoing prescriptions for medicines sold under different brand names in Spain (always carry the generic INN name), and those in rural inland villages where the single pharmacy closes for two weeks every August with no local alternative. In that last case the guardia rota covers a wider zone and you may be driving 15 to 25 km for a night-time receta.

The pharmacy question matters when you choose a town. For a full-time resident with chronic medication, living in Denia, Torrevieja, Benidorm or Alicante means a pharmacy within walking distance at almost any hour. In a hillside urbanisation above Polop or on a Finestrat golf resort, you are driving to the nearest village. Our guide to living in Denia in 2026 and the breakdown of who buys, rents and leaves Torrevieja both touch on this density advantage.

FAQ

Can I use a prescription from my home country in a Spanish farmacia? For EU and EEA countries, yes, if the prescription follows the cross-border format (prescriber name, patient details, INN drug name, dosage). UK NHS prescriptions are not accepted post-Brexit. A private UK prescription is accepted at the pharmacist's discretion for non-controlled medicines, but most will ask you to visit a Spanish GP first.

Do Spanish pharmacies take card payment? Yes, universally. Contactless and mobile payment are standard. Cash is still common for small purchases but no farmacia will refuse a card.

Is there a 24-hour pharmacy in every coastal town? Every municipality has guaranteed overnight pharmacy coverage through the guardia rotation, but it is not always the same shop and it may not be the one nearest your home. In small villages, the duty pharmacy can be in the next town. Save the Colegio Oficial website for your province to your phone.

Are Spanish pharmacy prices regulated? Prescription medicine prices are set nationally by the Ministerio de Sanidad and are identical in every farmacia. Over-the-counter drugs, cosmetics and parapharmacy items are free-pricing, so a bottle of sunscreen can cost 30 percent more in a tourist-strip pharmacy than in a Mercadona.
